Custody Authorizations

Custody Authorizations are generated in the Office of Student Services and Security. Guardians can contact the Office of Student Services and Security to learn more about the need for permits for students who have custody changes, placement changes, and other reasons. Additionaly, the form below must be completed for the Office of Student Services and Security to complete the Custody Authorizations 330-761-2742.
